Eye Consultants' "I Want Free iLASIK!" Video Contest
Contest Rules:
Contestants must be over 18 years old to submit a video to the contest. Videos may be submitted on behalf of another person, but both the entrant and the potential winner must be over the age of 18. Contestants may enter only one video to qualify. A contestant consists of a single person (no group submissions). Individuals who have previously had refractive surgery, to include LASIK, PRK, RK, Phakic IOLs or other procedures, may not submit a video for themselves. Videos submitted in previous contests may not be resubmitted for the current contest.
Those wishing to participate must first register for the contest at www.LASIKSpokane.net and follow all instructions provided for submitting videos. Neither Eye Consultants, nor KZZU, will provide technical assistance for the production or initial posting of video submissions. Videos must be submitted no later than November 6, 2011 to be considered for the contest. The winner will be announced on November 11, 2011.
Videos must meet the following criteria to be considered:
- No more than 4 minutes in length
- Videos can be either live-action or animated
- Must not contain copyrighted or otherwise protected material (including video or audio clips, background music, etc.)
- Content must be appropriate for family viewing
- Individuals must post the video submission on their own YouTube channel as indicated in the submission instructions. Videos cannot be mailed or hand delivered.
Videos will be judged for creativity, quality and content by a panel of judges consisting of Eye Consultants and KZZU staff and other invited guests. Panel members, collectively and individually, reserve the right to disqualify and remove any video that they deem to be inappropriate in nature.
By submitting video entries, contestants relinquish ownership rights of all associated content. All video entries will be considered the property of Eye Consultants. Eye Consultants reserves the right to download or copy any submitted video for use during the contest, and to use submitted material after the contest has ended in any way that it chooses.
The grand prize of free iLASIK consists of the following:
- Free pre-operative testing to determine candidacy for procedure
- Free IntraLASIK procedure (iLASIK) performed on both eyes. Procedures performed by Gary Fillmore, M.D., a Board Certified Ophthalmologist (Eye Physician and Surgeon)
- All necessary post-operative care for up to one year after the procedure is performed
The grand prize has NO CASH VALUE, but is transferrable. Not everyone is a good candidate for iLASIK. The grand prize winner must be determined eligible for iLASIK in order to accept the prize. The determination of candidacy for the procedure rests solely with Gary Fillmore, M.D. If the grand prize winner is excluded as a candidate for the procedure by preoperative evaluation and testing they may transfer the prize to another person of their choosing. If the grand prize winner does not wish to transfer the prize to another person, the panel of judges will review the remaining videos and select a new grand prize winner.
The grand prize of free iLASIK is not valid for, or in conjunction with, any other vision correction procedure performed by doctors at Eye Consultants.
Registration Terms and Conditions:
By registering for the Eye Consultants "I Want Free iLASIK!" video contest at www.LASIKSpokane.net, contestants agree to receive promotional materials from Eye Consultants.
Eye Consultants will not sell, lease or distribute contact information to any outside parties. Eye Consultants will use contact information for the sole-purpose of communicating contestant notifications, and offering special promotions and incentives from Eye Consultants and the EyeTech Laser Center. Opportunity will be provided with any e-mail communication from Eye Consultants to unsubscribe from ourcontact listings, thereby discontinuing any further e-mail communication.
